I understand and unconditionally and irrevocably accept the below terms.
- Should I re-schedule my/my son's surgery within one week of the scheduled surgery, this deposit is forfeited.
- Should I cancel my/my son's surgery without an approved medically acceptable reason of the scheduled surgery, this deposit is forfeited.
While this may appear to be a charge for services which were not provided, this fee is necessary to reserve time in the practice, which is done when I schedule.
