I have read and agree to the Terms & Conditions.

I have read and agree to the Privacy Policy.

I understand and unconditionally and irrevocably accept the below terms.

• Should I re-schedule my/my son's surgery within one week of the scheduled surgery, this deposit is forfeited. 

• Should I cancel my/my son's surgery without an approved medically acceptable reason of the scheduled surgery, this deposit is forfeited.

While this may appear to be a charge for services which were not provided, this fee is necessary to reserve time in the practice, which is done when I schedule.
