How Do I Look After Myself Post Shang Ring Circumcision?

Wearing the Shang Ring:

  • After the operation, rest at home for two days.
  • Do not try to remove or reposition the Shang Ring.
  • Do not shower/bath in the first 24 hours.
  • You may apply gauze with Vaseline spread on it to avoid the Shang Ring from sticking to your underwear. Only do this for 4 days.
  • Always keep the genitals clean and dry.
  • 7-10 days’ post circumcision please present to our clinic to remove the outer ring.
  • Do not take part in intense activities, to avoid dislocating the ring.
  • Do not have sex or masturbate for 8 weeks.
  • If you have pain post circumcision, you may use the medications prescribed to you. It is normal to have painful erections whilst recovering. You may ask your pharmacist for pseudoephdrine or phenylephrine tablets to minimise night time erections. This should only be taken at night and only suitable for adults.
  • You may experience swelling around the Shang Ring. This is normal, so long as it does not cause you too much discomfort. Keeling the penis facing up will minimise the swelling.

Presenting to the clinic to remove your Shang Ring:

  • Please apply nummit cream above and below your Shang Ring 30 minutes before presenting to the clinic. Please apply glad wrap over the Shang Ring.
  • Please take Paracetamol (Panadol) and Ibuprofen (Nurofen) 1 hour prior to presenting to the clinic. If you are not driving you can take a dose of Oxycodone (Endone) 1 hour prior to presenting to the clinic.

After removing the Shang Ring:

  • Do not have sex or masturbate within the eight weeks post circumcision. The World Health Organisation recommends that you use condoms to protect the circumcision wound (as well as to reduce the transmission of HIV and other infections) for six months’ post circumcision.
  • Expect to have yellow granulation tissue forming with a black ring around the penis. Occasionally you may develop a bad smell in week 3 post circumcision.
  • If you develop increased pain, swelling or redness please contact our clinic.
  • Expect the complete healing of the wound to take 6-8 weeks

When to come to the clinic or message us:

  • If you notice bleeding from around the Shang Ring or from the wound after the circumcision or the Shang Ring removal.
  • If you experience difficulty passing urine.
  • If you experience seriously painful swelling, increasing redness, bruising, fever or discharge/appearance of pus.
  • If the Shang Ring is damaged or comes off accidently (partially or completely)

When can I shower?

You may shower after 24 hours.

When does the Shang Ring come off?

It comes off at about day 7. The outer ring is removed. The inner ring is also sometimes removed.

If the doctor decides to leave the inner ring, it will fall off itself within 1 week.

How often do I apply gauze on my penis?

Apply gauze on your penis for 4 days. Change the gauze every time you go toilet. Apply petroleum jelly (Vaseline) to the gauze. Ensure the Vaseline side of gauze faces the penis.

I have painful erections. What can I do to stop this?

You may take Phenylephrine or Pseudoephedrine to reduce the intensity of erection. Confirm with your pharmacist if this is safe for you. These medications can raise your blood pressure so it is not suitable for men who have high blood pressure.

I have bruising on my penis Is this normal?

Yes, this is normal. This is from the penile injections.

I have yellow forming is this normal?

This is normal. It is a combination of healing skin and dead skin. Please send a photo to confirm. This will be gone by the end of week 3.

I have a smell forming. Is this normal?

This is normal. It is from dead skin pealing away. It will be gone by the end of week 3. Avoid wearing shorts so other people don’t smell it.

My penile glans (tip of penis) is very sensitive. What can I do about it?

You can apply fragrant free moisturiser to the glans penis multiple times per day. You can apply cling wrap over the glans so the underwear does not absorb the moisturiser.

How long do I use the pain killers?

Your first line pain killers are Paracetamol and Ibuprofen. If they are not strong enough, you can use the prescribed medication. Use the pain killers every 4 hours when required. You can use all 3 medications simultaneously. The prescription pain killer can cause constipation and drowsiness.

You are unable to drive or use heavy machinery whilst you use the prescription pain killer.

How long must I refrain from sexual activity?

No sex or masturbation for 8 weeks. If you are at higher risk of acquiring HIV (this includes men who have sex with men), you must wear a condemn for 6 months post circumcision. The penis has a high density of HIV receptors.

When do I expect to see the final result?

Complete cosmetic healing occurs at 8 weeks. In younger boys it can occur in 3-4 weeks. The younger the male the quicker the healing.

When can I resume sport?

You can resume sport in 3 weeks.

You may resume swimming at a swimming pool when the wound closes up.

When can I return to work?

If you work from home you can work the next day.

If you work in the office, you may return in 1 week.

If you are physically active at work (e.g. tradie), you may resume work in 10 days.
